What makes a record ratifiable: wind, timing and measurement
A performance becomes a record only once a set of conditions has been verified, and most disallowed marks fail on something the athlete never controlled.

Why ratification exists
A record is a comparison across places and eras, so it only means anything if the conditions behind each mark were broadly comparable. Ratification checks that a performance occurred under conditions the sport has agreed to treat as equivalent to every other. This is why a performance can be genuine, witnessed and accurately timed and still fail to be accepted as a record.
The checks are procedural rather than sceptical, and they apply identically to every mark that is submitted. Understanding them removes most of the confusion when an outstanding performance is not formally recognised.
Wind and where it matters
A following wind assists sprints, hurdles and horizontal jumps materially, so a limit applies to each of those events. Above the limit a performance stands as a result but cannot be ratified as a record, which is why marks are published alongside wind readings. The gauge must be positioned and operated to specification, and a missing or invalid reading is itself enough to prevent ratification.
Events run around a full lap are not wind-limited, because a runner faces the wind in both directions across the distance. That asymmetry explains why some events carry a reading beside every mark and others carry none at all.
Timing systems
Records require fully automatic timing, started electrically by the gun and stopped by a photo-finish image rather than by a person. Hand timing is accepted for many purposes but never for records, because human reaction introduces a systematic bias in one direction. The photo-finish records the torso crossing the line, which is why an outstretched arm or trailing leg does not count.
Timing resolution is specified, and results are rounded in a defined direction so that a mark is never flattered by the rounding. These requirements are the reason a stopwatch time, however carefully taken, can never become a record.
The facility itself
Track dimensions are certified, and a lane measured even slightly short would render every time recorded on it incomparable. Road courses require certified measurement by a specified method, and start and finish must satisfy limits on separation and on elevation drop. The drop limit exists because a net descent assists a runner across a long distance in a way no other course feature does.
Separation limits address wind, since a point-to-point course can enjoy assistance along its entire length rather than half of it. This is why certain fast road courses cannot produce ratifiable marks despite being measured with complete accuracy.
Eligibility, equipment and the paperwork
The athlete must have been subject to the applicable anti-doping requirements, including testing around the time of the performance. Equipment must comply with the rules in force, and footwear specifications have become a significant part of that check. The competition itself must have been sanctioned and conducted under approved rules with appropriately qualified officials present.
A complete application requires documentation of each of those elements, which is why ratification takes as long as it does. Marks are occasionally rejected on administrative failures alone, which is frustrating but follows directly from treating the conditions as part of the record. The record book is therefore a list of verified circumstances quite as much as of performances, and a great many outstanding marks sit outside it for reasons unrelated to how they were run.
